OTTAWA - A report by the watchdog over Canada's prison system says the death of a 19-year-old inmate was preventable.

Howard Sapers says a host of serious failures led to the death last October at the Grand Valley Institution for Women in Kitchener, Ont.

He is urging Public Safety Minister Stockwell Day to take several steps to prevent similar deaths.

The report delivered to Day today will not be made public while a criminal investigation continues.

Ashley Smith died in an apparent suicide in a segregation cell after she was transferred 17 times between institutions.

Sapers says she did not receive the mental-health support that she desperately needed.
